BEVERLY HILLS, CA — As social media continues to fuel demand for sculpted, contoured facial features, buccal fat removal has become one of the most talked-about cosmetic procedures. Board-certified facial plastic surgeon Dr. Babak Azizzadeh recently shared his expert perspective on the surgery, emphasizing the importance of a customized approach to achieve natural, long-lasting results.
Buccal fat pads, located in the lower cheeks beneath the cheekbones, play a functional role in chewing and help define facial fullness. While the size of these fat pads varies from person to person, they tend to remain relatively stable throughout life. For individuals with naturally fuller cheeks who desire a more sculpted appearance, buccal fat surgery offers a potential solution.
However, Dr. Azizzadeh cautions that removing buccal fat isn’t always the best approach. His preferred technique involves repositioning rather than simply extracting the tissue. “Instead of removing it, my technique is to move it to other areas that have lost volume over time and suspend it into place, such as in the upper cheek and deep nasolabial fold,” he explains. This method helps restore balance to the face while avoiding the hollow, gaunt appearance that can occur when too much fat is removed.
The procedure itself is minimally invasive, typically performed under local anesthesia through incisions inside the mouth, leaving no visible scars. Recovery is generally brief, with most patients returning to normal activities within days. Still, full results may take several months to become apparent as swelling subsides.
Dr. Azizzadeh stresses that candidacy depends on several factors, including facial structure, skin elasticity, and individual aesthetic goals. A thorough consultation helps determine whether buccal fat repositioning, removal, or an alternative treatment is most appropriate for each patient.
