Preparation

For the best results and swiftest recovery, we recommend the following leading up to your procedure.

Additionally, make arrangements for time off work and plan for any necessary post-operative care. Ask Dr. Azizzadeh about discontinuing non-surgical procedures such as botulinum toxin, fillers, biostimulators, and lasers.

Obtain any necessary pre-operative medical clearance and tests, such as bloodwork (e.g., complete blood count and coagulation profile), and an EKG or chest X-ray if needed based on age or medical history. Avoid taking aspirin, anti-inflammatory medications, and certain herbal supplements that can increase bleeding risk. This is also a good time to handle personal grooming, such as scheduling hair appointments, since certain activities may be limited post-surgery.

If you are undergoing revision rhinoplasty, Dr. Azizzadeh may recommend starting antibiotic ointments in nostrils and nose a few days prior to surgery.

On the night before your surgery, avoid eating or drinking after midnight if general anesthesia is to be used. Practice relaxation techniques to ease any pre-surgery anxiety and aim for a full night’s rest. If prescribed, take any anti-nausea medication as directed. On the morning of surgery, shower with antibacterial soap (if instructed), and do not apply any makeup, lotions, or skincare products. Wear loose-fitting clothing that buttons or zips in the front and follow any specific instructions regarding preoperative medications. Arrive at the surgical facility at the designated time, usually 1 to 1.5 hours before your procedure.

The initial recovery period following revision rhinoplasty typically lasts one to two weeks. During this time, you may need to take seven to ten days off work, depending on the extent of bruising and swelling. A splint will be placed on your nose immediately after surgery and will be removed one week later. In most cases, the dreaded “nasal packing” is not required, and in fact Dr. Azizzadeh may use a nasal device to facilitate better breathing during the first week of recovery.

Revision Rhinoplasty is a long-term commitment that involves ongoing care and routine follow-ups. While initial improvements will be visible after the swelling subsides, the full results of the surgery may take two to three years to fully manifest as the healing process completes. Dr. Azizzadeh prioritizes staying connected with his patients throughout this journey, scheduling follow-up visits every three months during the first year and annually thereafter to ensure your progress is on track and to address any concerns as you heal. Recovery from rhinoplasty can be divided into several distinct phases:

First 10 Days: During this initial phase, swelling and bruising around the nose and eyes are most prominent but will gradually subside. You may experience some discomfort, nasal congestion, and temporary changes in sensation, such as numbness or tingling around the nose. Rest is crucial, but gentle activities, like short walks, can promote circulation and aid in recovery. By the end of this period, many patients can resume light daily activities and return to work, although strenuous activities should still be avoided.

Days 10 to 30: Swelling and bruising continue to diminish, and you may feel more comfortable engaging in social activities as your appearance becomes more natural. Some residual swelling, particularly at the tip of the nose, may persist but is usually subtle. Most patients can gradually resume normal exercise and more strenuous activities around the three-week mark, as directed by their surgeon.

Up to 6 Months: The majority of the swelling should resolve during this period, and the nose will begin to settle into its more refined shape. You may notice continued subtle changes as the nose heals. Any lingering numbness or tightness around the nose typically improves as the healing process progresses.

Up to 2-3 Years: Full healing and final results from revision rhinoplasty can take up to two to three years. During this time, any remaining swelling, particularly at the nasal tip, will subside, and the nose will achieve its final contour and definition. Consistent follow-up with your surgeon is important to monitor your progress and address any questions or concerns throughout the recovery process, as individual healing can vary.

Revision Rhinoplasty Aftercare

In the first 72 hours after surgery, use cold compresses for 20 minutes per hour while awake to reduce swelling and bruising. After this initial period, switch to warm compresses applied for 20 minutes, three times a day, to help alleviate bruising and promote healing. You will also be given arnica and bromelain to help reduce swelling and bruising—arnica is known for its anti-inflammatory properties, and bromelain, an enzyme found in pineapples, helps decrease swelling. A dose of steroids will be administered during surgery to minimize swelling, and in some cases, a short course of steroids may be prescribed post-surgery to manage excessive swelling. This regimen aids in managing discomfort and speeding up the recovery process.

In the first 72 hours after surgery, use cold compresses for 20 minutes per hour while awake to reduce swelling and bruising. At the time of surgery, you will also receive a dose of steroids to minimize swelling. In some cases, a short course of steroids may be prescribed after surgery to manage excessive swelling. This regimen helps manage discomfort and accelerates the recovery process. 

Thanks to modern anesthesia methods and our specialized protocol, post-operative nausea is much less common and very manageable. Your anesthesiologist will administer intravenous anti-nausea medications and specialized anesthetics during surgery to limit gastrointestinal issues. Additionally, you will be prescribed Emend™ to take three hours before surgery to prevent nausea and Ondansetron (Zofran®) for any breakthrough discomfort. Avoid complex or heavy foods for 24 to 48 hours after surgery. Start with clear liquids and bland, easy-to-digest foods such as broth, plain toast, or crackers, and gradually reintroduce solid foods to ease digestion and minimize discomfort. Hydration is crucial during the post-operative period. If submandibular gland modifications were performed, follow a bland diet, avoid spices, and choose soft foods to minimize discomfort and swelling in the neck area.

For the first week, gently clean your rhinoplasty incisions with a solution of half water and half hydrogen peroxide once a day. After cleaning, apply an antibiotic ointment, such as Neosporin or Mupirocin, over all incisions three times daily to prevent infection and promote healing. After the first week, discontinue hydrogen peroxide and antibiotic ointment. Steri-strips or silicone gel will be recommended to support further healing. If you notice crusting, applying Vaseline can help keep the skin moist.

Keep your head in a neutral and elevated position for at least one week following surgery. This includes when you sleep, rest, or relax.

You can begin taking short walks during the first week after surgery and gradually increase your activity level. Strenuous activities can typically be resumed three to four weeks post-surgery, but you should receive clearance from Dr. Azizzadeh before engaging in any intense physical activities.
